Fire Application
Step One
Download application PDF to your computer and fill it out. You must have a program like Adobe Reader installed to fill out the PDF and save it (DO NOT use your browser’s PDF viewer to fill out the application, for you will not be able to save it).
Save the PDF onto your desktop folder for easy access.
Step Two
Open the file from within your desktop folder and fill out your application. Once you are finished, save your file.
Upload your completed application PDF along with any supporting documents using the form below. IMPORTANT: Remember to save your changes on the application before uploading it.
